What is prostate enucleation with Holmium laser or HoLEP?

Holmium laser enucleation of the prostate with Moses effect, is minimally invasive surgical procedure indicated for treating benign prostatic hyperplasia, one of the most common conditions in men.

This innovative surgical approach allows us to treat large prostates (greater than 80 grams) through the urethra, which traditionally would have been operated on using an open approach (retropubic adenomectomy).

It is currently considered the technique of choice because it has the same results as conventional surgery, but minimises bleeding and, consequently, reduces hospitalisation time and overall recovery time.

What advantages does Holmium laser enucleation have?

  • There is no incision on the skin, and therefore, no external wound.
  • Bleeding and, consequently, the risk of transfusion is minimal.
  • Holmium laser prostate enucleation (HoLEP) does not cause erectile dysfunction, changes in sexual desire, or affect orgasm.
  • Shorter time with a urinary catheter (24-48 hours).
  • Shorter hospital stay (no admission required).
  • Quick return to normal activities.
  • Allows for concurrent treatment of bladder tumors, urethral strictures, and bladder stones.
  • Tissue is available for histopathological analysis (biopsy).

Who is the prostate laser recommended for?

 

The laser is indicated for men with moderate to severe Benign Prostatic Hyperplasia (BPH), such as interrupted urination, the sensation of incomplete emptying, decreased urinary flow, frequent urination at night, or acute urinary retention.

It is specifically designed for patients with very large prostates who wish to avoid major surgeries and seek long-lasting results.

How is Holmium laser prostate surgery performed?

The surgery is performed entirely through the urethra in two phases:

  • Enucleation: The laser energy cuts and separates the prostatic tissue that obstructs the flow from the prostatic capsule.
  • Morcellation: Using an instrument that simultaneously shreds and aspirates, all the enucleated prostatic tissue is extracted through the urethra. This allows for the collection of prostatic tissue for histopathological analysis (biopsy).

How long does the procedure last?

The procedure lasts 60 to 120 minutes (depending on the size of the prostate). The Moses technology reduces the intervention time by 25 minutes compared to traditional Holmium laser.

 

At what prostate size is Holmium laser recommended?

It is especially recommended for patients with a large prostate volume greater than 80 grams to ensure good results.

What is recovery like?

90% of procedures with the Moses effect are outpatient.

The patient is discharged with a urinary catheter that remains in place for 24 to 48 hours to prevent complications and discomfort.

After the catheter is removed, the patient will immediately regain normal urine flow.
