What is hypogonadism or testosterone deficiency?
Male hypogonadism occurs when the testes produce insufficient testosterone or when there is a regulatory dysfunction that causes low blood levels. It can be:

70–80 years: 30%

80 years: 45%
In men with obesity, type 2 diabetes or metabolic syndrome, prevalence can be 2–3 times higher.

Symptoms of hypogonadism (low testosterone)

Causes and risk factors
Although the exact causes of this deficiency are unknown, some of the risk factors that influence it are:

Aging, obesity, metabolic syndrome.

Untreated sleep apnea.

Congenital or acquired hypogonadism.

Previous chemotherapy, radiotherapy, chronic diseases.

Drugs (opioids, steroids, anabolic agents).

Chronic stress and poor sleep habits.
How do we diagnose hypogonadism?

Clinical history and symptom questionnaires.
Morning blood tests (08:00–11:00), repeated twice:
- Total testosterone.
- SHBG and albumin to calculate free testosterone.
- LH, FSH, prolactin, thyroid and metabolic profile.
Study of reversible causes.

How is hypogonadism treated?

- Transdermal gels.
- Intramuscular or subcutaneous injections.
- Subcutaneous pellets.
For men who wish to preserve fertility, alternatives like SERMs, HCG or aromatase inhibitors can be considered, either alone or combined with TRT.
