What is premature ejaculation?

Ejaculation is a reflex process in which there is a point at which control is lost, known as the ‘point of no return’. However, before reaching this point, the person may have some control. Recognising this point and understanding the phases of the sexual process are essential for effectively controlling ejaculation and improving the prognosis.

Premature ejaculation occurs when it happens earlier than desired, usually within a short period of time and with a feeling of lack of control over the act. This can cause significant distress and often leads to avoiding shared sexual relations or experiencing them in an unsatisfactory manner. This set of circumstances negatively impacts the individual’s self-esteem and perception of sexual competence.

What types of therapies are available for premature ejaculation?

This problem affects up to 25% of the adult population, regardless of age, and in approximately 90% of cases it is related to psychological or emotional issues.

Given that most cases are psychological in origin, sex therapy offers multiple tools to address this problem. The prognosis is generally positive and treatments are usually brief, but patient commitment is essential for therapeutic success.

Pelvic floor strengthening (Kegel exercises)

Kegel exercises teach you how to strengthen and train your pelvic floor, which allows you to achieve better quality erections, more intense orgasmic contractions and, therefore, a greater perception of the quality of sexual encounters, as well as preventing issues such as urinary incontinence and achieving greater control over the ejaculatory reflex.

The Kegel technique consists of voluntarily contracting and relaxing the pelvic floor muscles in different sets and at different speeds.

Self-stimulation training

The ejaculation reflex can be trained through masturbation using the pinch technique. This involves applying pressure to the frenulum when you are very aroused, but not at the point of ejaculatory inevitability, because at that point there is nothing you can do to control ejaculation.

The aim of this technique is to realise that you can lose your erection and that, with the right stimulation, you can regain it while extending the time to ejaculation and gaining greater self-confidence.

Breathing exercises

When we are aroused, our breathing rate increases (we breathe faster). Gaining greater control over our breathing allows us to prolong ejaculation times. Through guided breathing, we learn to identify the signs of arousal and focus our attention on inhaling and exhaling more slowly, thereby delaying ejaculation.

Combined couples therapy

At first, it is common for ‘sexual intercourse to be prohibited’. Non-genital sensory focus exercises are performed to familiarise patients with their body’s responses to stimulation and to identify each patient’s erogenous zones. Patients are trained to slow down and focus on the pleasurable sensations produced by bodily stimulation, leaving stimulation of the genital area until last.
